Indicate the Type of Records You Wish to Request
Upload a Protected Health Information Form (you have the option of using our form with any subscription)
Upload a Copy of Your Client’s Identification

You will receive weekly updates sent to the email address you provided during your request.
Please watch for the following important emails:
Confirmation that your request has been received by the facility.
Confirmation that the invoice has been issued by the facility.
Confirmation that the records have been dispatched by the facility.

Once we receive confirmation from the facility that the records have been dispatched and you confirm their receipt, we will update the status of the request to complete.
